What Is Forward Head Posture?

Forward Head Posture is a postural pattern in which the head rests in front of the shoulders instead of aligning directly above the upper spine. This position can increase strain across the neck, upper back, and supporting muscles. The pattern may develop during prolonged screen use, desk work, driving, or other activities that keep the head angled forward.

Key Takeaways About Forward Head Posture

  • The head sits ahead of the shoulder line.
  • Daily screen and desk habits can reinforce the pattern.
  • Forward Head Posture is not the same as a rounded upper back.

How Forward Head Posture Works

A man in a gray shirt sits indoors while another hand rests on his shoulder

Forward Head Posture changes the relationship between the head, neck, shoulders, and upper back. As the head moves ahead of the shoulder line, the neck muscles must manage a different load during standing, sitting, walking, and screen use. The upper back may also adjust its position to help keep the eyes level.

A posture assessment looks at head and shoulder alignment from the side and considers movement during ordinary tasks. The assessment may include observation of the cervical spine, shoulder position, and upper-back movement. Posture correction may then address daily habits, joint motion, muscle control, and workstation position. The goal is to improve how the body coordinates these parts during useful activities.

From the Advanced Injury Care Clinic team

When we assess Forward Head Posture, we observe head and shoulder alignment during standing and movement. We also consider neck motion, upper-back position, and the daily activities that may reinforce the pattern. Our Posture Correction planning connects these findings with practical changes that fit the person’s routine.
